Rummy Day was yesterday, but the record, I'm essentially certain that Rumsfeld's famous Known Unknowns vs Unknown Unknowns comes from this presentation by Barnett. Rumsfeld almost certainly saw it, whether in the private or public sector at the time.

I've used that with attorneys since before I worked for one. Ask if the attorney can recommend an attorney specializing in a legal field. The attorney will then ask about your issue and, in all likelihood, offer a solution.
Ask the attorney straight up and you'll get told to make an office appointment. Never ask a professional to provide you his or her expertise for free, but offer to pay a competitor for it and you'll get it for free.

In August 2015, geniuses at Savannah River Site decided to substitute commercial software for the custom Document Control Register - to "save money".
They broke 15 years'-worth of links to it from e-mails written by the entire Engineering group, Operations and Training. Just one job, that of Impact Evaluator (tracks regulatory procedure changes when equipment is replaced), now requires seven people. So much for savings.
6/1/21 saw a Federal requirement take all emails older than 7 years to the trash. The facility is 60 years old...
They switched to Outlook via Exchange from the completely on-site Notes, and defended offloading Site e-mail to Microsoft servers; not only can we not use email at all if the link is down, Exchange got hacked in May.
So sometimes the loss is deliberate, the actions of the truly stupid.
